Students with asthma should have a yearly check up with their health care providers. If a student has had symptoms or required asthma treatment within the past year, an Asthma Action Plan (below) should be filled out and signed by your child's provider. Please see the nurse if your child needs to keep an inhaler at school.
Severe Allergies
The school nurse should be notified of all food/other serious allergies and intolerances. Severe allergies, with the potential for a life threatening, anaphylactic reaction, requires an emergency action plan and emergency medications such as an Epi-pen to be kept at school. Your health care provider must fill out and sign an Allergy Action Plan (below) and be updated with the nurse annually. Please see the nurse for more information.
Other Serious Medical Conditions
Students with other serious medical conditions may require an Emergency Action Plan. Please speak with the school to discuss this further.
